One way for persons with disabilities to apply for eligibility under the Americans with Disabilities Act (ADA) is via the following forms:
Both portions of the applications (the applicant's portion and the professional verification) must be received by The Rapid before the ADA eligibility review process begins. The application portion, completed by you or your support person, must be submitted online after completion.
The Professional Verification form must be filled out by a licensed social worker, psychologist, nurse, physician, or rehabilitation specialist who is familiar with your most limiting condition(s). They can print out and mail the completed professional verification to:

If you cannot apply online, or if you are applying for the over-age-65 rate, you can get application forms by calling the GO!Bus office at 616-456-6141, 8:00 a.m. to 4:30 p.m., Monday through Friday.
The Rapid is required to make a determination of ADA eligibility within 21 days of receiving a fully completed application. A fully completed application comprises both the applicant's portion AND the professional verification. If the ADA eligibility determination has not been made within 21 days, the applicant is presumed eligible and can use the GO!Bus service until eligibility is decided.
