How to Apply
One way for persons with disabilities to apply for eligibility under the Americans with Disabilities Act (ADA) is via the following forms:
Both portions of the applications (the applicant's portion and the professional verification) must be received by The Rapid before the ADA eligibility review process begins.
This form can only be submitted electronically by the applicant or the applicant’s legal guardian. No hard copies will be accepted. The Professional Verification form must be filled out by the physician, psychologist, or rehabilitation specialist who is verifying your disability. The Professional Verification form may be printed out to give to the professional and then returned to The Rapid by mail.
If you cannot apply online, or if you are applying for the over-age-65 rate, you can get application forms by calling the GO
The Rapid is required to make a determination of ADA eligibility within 21 days of receiving a fully completed application. A fully completed application comprises both the applicant's portion AND the professional verification. If the ADA eligibility determination has not been made within 21 days, the applicant is presumed eligible and can use the GO
