The Professional Verification form must be printed out and filled out by the physician, psychologist or rehabilitation specialist who is verifying your disability. Non-disabled seniors do not need professional verification. How Long it TakesThe Rapid is required to make a determination of eligibility within 21 days of receiving a fully completed application.
