How to Apply

One way for the disabled to apply is by using our on-line form below. This form can only be submitted electronically by the applicant or the applicant’s legal guardian. No hard copies will be accepted. The Professional Verification form must be filled out by the physician, psychologist or rehabilitation specialist who is verifying your disability. The Professional Verification form may be printed out to give to the professional and then returned to The Rapid by mail.

If you cannot apply on-line, or if you are applying for the over-65 rate, you can get application forms by calling the GO!Bus office at (616) 456-6141, 8 a.m. to 4:30 p.m., Monday through Friday.

How Long it Takes

The Rapid is required to make a determination of eligibility within 21 days of receiving a fully completed application. If the determination has not been made within 21 days, the applicant is presumed eligible and can use the GO!Bus service until eligibility is decided.
